Ethics Complaint Response Form

This form shall be used by an individual who is the subject of a Via Hope Ethics Complaint to provide an official response to the details and alleged violations in the initial Complaint. Via Hope will use the information provided to gather additional information, and the completed Response Form will be sent to the Advisory Council along with any other relevant documentation. The Response Form and accompanying documentation will be redacted for any identifying information of the individual filling out the Response Form or any witnesses or other individuals listed. This form must be returned within ten (10) business days of receipt or the individual’s intent to provide a response will be considered waived.
