Developed by the Yale Program for Recovery and Community Health (Yale PRCH), Getting In the Driver’s Seat of Your Treatment: Preparing for Your Plan, is a tool that is intended to be used in partnership persons served by mental health provider organizations in preparation for a person-centered planning process.
Created for use in Connecticut, this tool has been used by the coordination team and sites involved in the 2011 Recovery-Focused Learning Community and the Recovery Institute.
